Inspector Notes

An unannounced subsidy health and safety inspection was conducted on 02/28/2023, between the hours of approximately 11:25 a.m. and 12:05 p.m. There was 1 children in attendance at the time of the inspection. An inspection of the physical plant was completed. Required background searches and household member information was reviewed. A sample of requirements for daily attendance, child information/records, emergency preparedness planning and supplies were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the vendor. Based on record review, the vendor did not obtain all information for children?s records.

Evidence: Child #1?s (start date: 10/24/2021) record did not include proof of the child?s identity.
Plan of Correction: I will ask the father for it.
Standard 8VAC20-790-260-A
Based on observation, areas of the family day home were not maintained in a clean, safe, and operable condition.

Evidence: There were multiple areas of chipped and peeling paint on the wall in the living room.
Plan of Correction: They are coming in April to paint.

Based on observation, in areas used by children of preschool age or younger, not all electrical outlets had protective covers.

Evidence: In the dining area, used by Child #1 (age: 3 years), there were three electrical outlets that did not have protective covers.
Plan of Correction: I will cover them today.

Based on observation, a crib did not meet the current Consumer Product Safety Commission Standards.

Evidence:
1. The vendor stated that the crib is used for a 10-month-old that is enrolled.
2. The crib provided by the vendor was manufactured on 10/27/2010.
Plan of Correction: I will get a new one this weekend.

Based on observation, medication was not kept in a locked place using a safe locking method that prevents access by children.

Evidence: One pain reliever and one allergy medication were found in the top unlocked drawers of a dresser located in the living room, accessible to children.
Plan of Correction: Corrected during the inspection.

Based on record review and vendor interview, shelter-in-place procedures were not practiced twice a year.

Evidence: The vendor stated that they did not practice shelter-in-place procedures in 2022.
