Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on 9/23/2025. The inspection started at 11:45am and concluded at 12:45pm. There were no children present. The inspector reviewed 1 child?s records and 1 caregiver/household member(s) records.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
