Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews (if applicable) and a virtual tour of the program (if applicable).

A subsidy health and safety inspection (SHSI) was initiated on August 31, 2021 and concluded on the same date. There were 2 children present with 1 staff supervising. A code compliance inspection was also conducted on this date.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the program.

Based on record review and interview, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child as it occurs.

Evidence: The vendor stated there are no attendance records. There were 2 children present at the time of inspection.
Plan of Correction: I will take attendance everyday.
Standard 22VAC40-665-170-B
Based on record review, the vendor did not ensure all children's records had all required information.

Evidence:
Child 1 does not have a written authorization for emergency medical care if an emergency were to occur.
Child 2 does not have documentation of proof of identity.
Plan of Correction: I will make sure the parent signs the authorization for emergency medical care and complete the proof of identity form.

Based on observation, cleaning products were stored in an area accessible to children.

Evidence: Cleaning products were observed on a waist high bookshelf. A school age child can reach the shelf easily.
Plan of Correction: I will put the cleaning products in a place children cannot reach.
