Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated and completed on 09/10/2025. The inspection started at 11a.m. and concluded at 12p.m. There were 1 child present and 1 caregiver; the point total was 4. The inspector reviewed 3 children?s records on-site household background clearances electronically.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
3The child's information shall include the addresses for two emergency contacts and proof of the child's identity.
Child #1, #2 and #3 records did not contain proof of the child's identity or addresses for 2 emergency contacts.
The provider has not completed the annual health and safety update course. The course was last completed over 16 months ago.
Shelter in place procedures were not practiced in 2024.
