During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
S subsidy health and safety inspection (SHSI) was initiated and completed on February 3, 2025. The inspection started at 10:05am and concluded at 11:30am. There was one child present and one caregiver; the point total was 3. The inspector reviewed one child's records and three caregiver/household members' records on-site. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Standard 8VAC20-790-180-B
The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. There were no attendance records maintained for over eleven months.
Plan of Correction: I will start keeping a daily attendance sheet.
Standard 8VAC20-790-230-B
The vendor shall obtain documentation of additional immunizations once every six months for children less than the age of two years. Child #1 who has been enrolled for over a year, did not have documentation of additional immunization. The last documentation for immunizations was dated December 2023.
Plan of Correction: I will call the parent and tell them to bring an updated immunization record. I will be sure to get immunizations records every six months for children under two.

Shelter-in-place procedures shall be practiced twice a year. No shelter-in-place procedures were practiced in 2024.
Plan of Correction: I will practice the shelter-in-place twice a year going forward.
