Inspector Notes

A complaint was received in the licensing office on 4/25/2024 regarding supervision. On 4/29/2024 an on-site investigation was conducted. Interviews were conducted, records were reviewed and observations were made. At the time of this investigation, the preponderance of evidence did not support the allegation and the complaint is deemed not valid. Based on a review of records and staff interview, a substitute provider did not record and sign the time of arrivals and departures on each day that the substitute provider worked.

Evidence:
1. The substitute Provider had been working at the Family Day home during the Provider's absence since 4/15/2024. As of the date of the inspection, which was 4/29/2024, there was no documentation of arrivals and departures on each day that the substitute provider had worked.
Plan of Correction: I will get the substitute provider log and log my hours each day.
