Inspector Notes

An unannounced renewal inspection was initiated on 12/3/2021 and concluded on 12/3/2021. There were 21 children present, with 10 staff supervising. The children were observed napping. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the tuberculosis (TB) screening/test form on file for a staff member did not include all required information.

Evidence:
Staff #1's record contains a TB screening/test form that does not include the date in which the test/screening was performed.
Plan of Correction: Current TB test was on file during the inspection. Center has obtained a copy of TB test with the date that test was performed.
