Inspector Notes

An unannounced monitoring inspection was conducted today from 11:40am-1:30pm. There were 27 children (infants-5yrs.) directly supervised by 6 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, eating lunch and napping. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Standard 8VAC20-780-130-E
Based on record review, the center did not obtain documentation of additional immunizations once every six months for all children under the age of two years.

Evidence:
Child #3's record contains an immunization record that was last updated on 3/2/2022.
Plan of Correction: The center obtained updated immunizations for the child with the date of 9/7/2022.
Standard 8VAC20-780-160-B
Based on record review, the tuberculosis (TB) screening/test form on file for a staff member did not include all required information.

Evidence:
Staff #3's record contains TB screening/test documentation that do not include the date in which the results of the screening/test were obtained.
Plan of Correction: The center received a new TB test from the employee. TB test was performed on 11/11/2022.
Standard 8VAC20-780-520-C
Based on observation, the center did not ensure all requirements were met when diaper ointment was used.

Evidence:
1. In the Infant room, two tubes of diaper ointment were not labeled with the child's name.
2. In the Toddler's room, one tube of diaper ointment was not labeled with the child's name.
Plan of Correction: The center has labeled all tubes with child's names in the Infant room and the Toddler room.
