Inspector Notes

An unannounced renewal inspection was conducted today from 2:00pm-4:00pm. There were 41 children (infants-school age) directly supervised by 6 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, napping, playing outside, and eating snack. There was an abundant supply of materials for the children. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Based on record review, the center did not obtain documentation of additional immunizations once every six months for all children under the age of two years.

Evidence:
Child #3's record contains an immunization record that was last updated on 2/3/2023.
Plan of Correction: Child #3 has a current/updated immunization record on file at the school.
Standard 8VAC20-780-160-A
Based on record review, the center did not obtain for each staff member documentation of a negative tuberculosis screening completed within the last 30 calendar days from their first day of employment.

Evidence:
Staff #5's file contained a copy of a Tuberculosis Screening form that was not dated.
Plan of Correction: Staff #5 has a current negative TB test on file at the school.
