Inspector Notes

An unannounced monitoring inspection was conducted 4/22/2024 from approximately 10:18am-12:00pm. Three classrooms were observed with total of 22 children directly supervised by 4 staff, all supervision ratios were in compliance. The physical plant, 2 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside out equipment, doing a table activity with Playdough, napping and listening to/singing along with music. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Based on observation, equipment of the center, was not maintained in a safe and operable condition.
Evidence: Red tube of playground equipment had two screws that were exposed and allowed enough space for fingers to be pinched or snag skin.
Plan of Correction: Director stated they would dispose of equipment.

Based on record review, documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering,
was not noted.
Evidence: Reference forms for Staff #1 were not dated.
Plan of Correction: Director will ensure date of reference will be noted on form.
