Inspector Notes
An in person monitoring inspection was conducted today. There were 6 children present (15 points) with 2 caregivers present. 5 child and 7 caregiver/household members records were reviewed. See Violation Notice for missing items. Outside time discussed with the provider: front yard use while a shed is being built, or a General Fieldtrip permission form would be required to take walks in the neighborhood. The inspection will be typed off site and emailed to the provider for signatures. LI was present today from 10:10 AM - 11:30 AM. *A tube of Aquaphor lotion in use for one child did not have a written authorization to apply a non-prescription topical skin product.

The freezer temperature registered at 3 degrees during the inspection.
