Inspector Notes

An in person monitoring inspection was conducted today. There were 7 children present (16 points) with 2 caregivers. The children were resting during the inspection.
3 child and 7 caregiver and household member records were reviewed.
Emergency evacuation drills, first aid supplies, emergency supplies, fridge temperature, and crib/cot spacing were all in compliance.

Standard 22.1-289.035-A
Based on a review of records, caregiver and household member records do not contain documentation of repeated background searches within the allowable 5 year time frame.
Evidence:
6 of 7 records reviewed did not contain documentation of repeated background searches within 5 years. Central Registry Searches (CPS) for caregivers and household members are all dated 1/11/17 (expired), (Caregivers A, B; Household Members C, D, E,F)
Plan of Correction: CPS documentation has been submitted. Awaiting results.
Standard 8VAC20-800-510-E
Based on observation, a cushion in use for rest is not able to be cleaned and sanitized.
Evidence:
The cushion in use is a soft sided, and not able to be cleaned and sanitized at least weekly.
Plan of Correction: I will replace cushion with cots and waterproof hard mattress.
Standard 8VAC20-800-530-A
Based on observation, rest mats/cots used by children did not have a top and bottom cover in use.
Evidence:
4 children resting on cots did not have a top cover in use.
1 child had no top or bottom cover.
Plan of Correction: I will order new covers to be used with cots.

Based on a review of records, child records did not contain documentation of acknowledgment of all required policies and procedures.
Evidence:
2 of 3 child records reviewed did not contain documentation of the decision to administer medication. (Children 1,3)
Plan of Correction: I will have parents sign a new copy.
Standard 8VAC20-800-940-A
Based on observation, the freezer was not maintained at or below 0 degrees F.
Evidence:
The freezer temperature registered 13 degrees F on the thermometer in the freezer.
Plan of Correction: I will adjust and monitor temperature.
