During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 02/19/2025, as a part of the licensure period. The on-site inspection began at 10:15am and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 3 children in care and 2 caregivers; the point total was 8. The inspector reviewed 3 children?s records and 2 caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children. Multiple items hazardous to children were observed in a closed, unlocked drawer below the sink in the bathroom, accessible to children.
Plan of Correction: We will lock the drawer.
Standard 8VAC20-800-340-C
The bathroom shall be kept clean and contain a working toilet and sink, toilet tissue, liquid soap, and paper towels. The bathroom in the family day home did not contain paper towels.
Plan of Correction: Corrected during inspection.
Standard 8VAC20-800-60-B
Children's records must contain all required information. The record for Child #1 was missing documentation of the child's first date of attendance, the employment information for the parents, the policy number of the child's medical insurance, and the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. The child was present during the inspection and had been in care for approximately four months.
Plan of Correction: We will request the information from the parents.
Standard 8VAC20-800-80-C
Providers are required to obtain from the parent verification of the identity and age of the child within seven business days and document the method of verification in the child's record. The records for Children #1, #2, and #3 did not contain documentation of the date the information was verified and the signature of the caregiver verifying the information. Children #1, #2, and #3 have been in care for approximately four months.
Plan of Correction: We will ask the parents to bring the documents in again and document viewing the documents.
Standard 8VAC20-800-830-B
Providers are required to practice shelter in place procedures with children in care a minimum of twice per year. The family day home did not practice shelter in place procedures in 2024.
Plan of Correction: We will make sure to complete two shelter in place drills in 2025.
