During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 02/19/2025 and completed on 02/26/2025 in response to a complaint received by the licensing office on 01/29/2025 relating to allegations of forbidden action. The inspector reviewed 2 children?s records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to emergency information and are documented on the violation notice issued to the program.

Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. Records for children #1 and #2 did not have a parent?s signature for an annual review of the emergency contact information. Child #1 and Child #2 had been in care for over a year.
Plan of Correction: Moving forward, we will make sure to have the parents review the information annually.
Standard 8VAC20-800-840-A
The provider is required to record in the child's record an injury or accident sustained
by a child while at the family day home that requires first aid. On 1/26/2025 Caregiver #1 observed bruising and swelling near Child #1's eye after naptime. Caregiver #2 applied first aid to the injured area. The record for Child #1 did not contain documentation of the injury.
Plan of Correction: Moving forward, we will make sure to always have blank copies and complete injury report forms for all injuries.
