Inspector Notes

An unannounced monitoring ins pection was conducted from 3:40pm - 4:15pm There were 8 children present and 3 staff members present. The sample size consisted of 3 children?s records and 3 staff records. Children were observed freely playing, singing and dancing.

Informatiopl gathered during the inspection determined noncompliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the licensee did ensure at least every two years from the date
of the first initial screening or testing staff members shall obtain and submit results of
tuberculosis (tb) screening.
