Inspector Notes

A subsidy health and safety (SHSI) inspection was completed on 12/4/2023 from2:30 pm - 5:00 pm. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
At the time of the inspector's arrival there were 11 children present, ages 4 to 5 years, with 2 staff supervising. Information gathered during the inspection determined violations with applicable standards or law. These violations are identified on the violation notice.

Based on record review and interview, the licensee dis not ensure that each staff annually attends the Virginia Department of Education's Annual Health and Safety Update Course.
Evidence:
Five staff records were reviewed.
The record for staff #4 documented her date of hire as 4.5.2022.
Staff #4's record did not have documentation of the Virginia Department of Education's Annual Health and Safety Update Course having been completed after one year of employment.
The program director confirms that she has not received documentation of staff #4 completing this training.
Plan of Correction: Staff #4 will complete this training by 1/4/2024.
