During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/6/202and completed on 8/8/2025. The on-site inspection began at 2:25 pm and ended at 4:05 pm. The inspector reviewed compliance in the areas listed above. There were 17children present and 1staff and a volunteer. The inspector reviewed 5 children?s records and 4 staff records, on-site and specific additional record items were viewed electronically on 8/8/2025. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #5, who had been enrolled for over 7 weeks.
Plan of Correction: This documentation may be available and will be submitted for review if it is located. If not, then the parent will be asked to submit results of a completed, signed physical examination record within 2 weeks.
