During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on April 1, 2026. The on-site inspection began at 9:52 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were 14 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review,
tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this
SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 9, 2026. A POC submitted after this date will not appear on the public website.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #1, ho had been enrolled for over 1 year.

Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Sanitizer were located on top of the staff desk at the center. Disinfectant wipes were underneath the staff desk at the center. Multi purpose cleaner was found outside nearby the outdoor equipment storage bin.
