Inspector Notes

An unannounced renewal inspection was conducted on October 30, 2023, from 9:30am to approximately 10:45am. There were three children present totaling seven points and the provider. The Inspector reviewed six children?s records and two staff records. Children were observed during arrival time to the daycare, indoor and outdoor play, and free play. Children and staff interactions were observed throughout the entire inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the provider at the exit interview.

Standard 8VAC20-800-100-A
Based on record review and interview, it was determined that the center did not ensure that the provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #1 (date of enrollment 04/10/2023) did not have documentation of a physical examination in their file.
2. Staff #1 confirmed that Child #1 did not have their documentation of a physical examination in their file.
Plan of Correction: The provider responded with the following: Staff #1 requested the documentation over the phone while during the inspection and will obtain the physical examination records from the parents.
Standard 8VAC20-800-90-A
Based on record review and interview, it was determined that the provider did not ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized.
Evidence:
1. Child #1 (date of enrollment 04/10/2023) did not have documentation of immunization records in their file.
2. Staff #1 confirmed that Child #1 did not have their documentation of immunization records in their file.
Plan of Correction: The provider responded with the following: Staff #1 requested the documentation over the phone while during the inspection and will obtain the immunization records from the parents.
