Inspector Notes

An announced monitoring virtual inspection was conducted on 6/24/21. Due to the COVID 19 Pandemic the inspector was not on site and this inspection was completed via video application as well as phone and email correspondence. There were 3 children (7 points) directly supervised by the provider. The home, staff records, children?s records, household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating snack. There was an adequate supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Based on review of household member documentation it was determined that all 18 year old's were not in compliance with background check requirements.

Evidence: During inspection Household member A who recently turned 18 did not have a completed Sworn Disclosure Statement and fingerprints.
Plan of Correction: I will get it done as soon as possible.
