Inspector Notes
A renewal inspection was initiated and concluded on March 4, 2024. There were two children in care with four teaching staff and two administrators. Children were observed in teacher and self directed activities including story, hand washing and transitions. Children and staff records were reviewed. This facility is reopening after being closed during COVID with further delays regarding staffing, staff training and renovations. Emergency evacuation and emergency equipment were reviewed. The facility is located inside a building on the Norfolk State University campus. The center consists of two classrooms with children's rest room facility located inside the child day center classroom suite. The outside play space is located next to the building with a walkway and grassy area between the two. There is climb and slide equipment and resilient surface present under and around equipment. During the exit meeting with the program director was informed that two violation were observed related to background checks. The violations appear on the violation notice.

1 - the records for Staff #2, #3 and #4 contained documentation that fingerprint background checks were obtained after the documented date of hire.
a. Staff #2 has a fingerprint background check dated January 2, 2024. However, the documented date of hire is November 10, 20203.
b. Staff #3 has a fingerprint background check dated January 2, 2024. However, the documented date of hire is November 10, 20203.
c. Staff #4 has a fingerprint background check dated December 29, 2023. However, the documented date of hire is November 10, 20203.
2 - Staff #1 confirmed that the fingerprint background checks were obtained after the documented date of hire.
Evidence:
The record for Staff #5 contains documentation that the staff lived in North Carolina within the past five years. However, there is no documentation that an out of state central registry background check was requested prior to employment.
