Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Caregiver #1 was not completed and was overdue by 23 days.
Plan of Correction: I completed the TB tests for myself and my husband on December 2. Our blood was drawn and the final reports will be sent to me soon. In the meantime, I have attached the proof of completion/approval that confirms our tests have been done.
My boys completed their TB screenings as well, and their results are negative. I have attached their approval paperwork.

'Areas and furnishings of the family day home, inside and outside, shall be maintained in a safe condition.

In the children?s nap room, there were two holes in the wall.
Plan of Correction: The hole in the room where the baby sleeps has been fully repaired. I have attached photos showing the completed work.

A coffee maker and a food processor were observed plugged in when not in use.
Plan of Correction: I have placed a clear note instructing everyone to unplug small appliances after use. I have attached a photo showing the posted note.
