The sworn statement for Staff #1, who is currently working, was dated eight days after their first day of employment.

1. Staff #1, who had been employed for seven months, did not have a central registry search requested. Staff #1 was working with children.
2. Staff #2, who had been employed for more than ten months, did not have a central registry search requested. Staff #2 was working with children.
3. Staff #3, who had been employed for four months, did not have a central registry search requested. Staff #3 was working with children.
Plan of Correction: Had to reach out to support for Central Registry. Each staff member has to be resubmitted.

A tuberculosis (TB) screening is required every two years from the last screening.

Staff #4 updated screening was due seven months ago. The updated screenings have not yet been completed.
Plan of Correction: Staff has an appointment for screening at Health Department.

There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

During the inspection on 1/13/2026, there was no staff member with current daily health observation training.
Plan of Correction: The daily health observation training will be done for every staff person.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Two cans of disinfectant with warning labels, ?Keep out of reach of children,? were unlocked in the infant classroom, out of reach of children.
Plan of Correction: Was corrected during inspection. Staff is aware the disinfectant should be locked in a cabinet. Hazardous substances should be in locked cabinet.
