Inspector Notes

An inspection was conducted 12/19/2023 based on the center?s 12/14/2023 report of an incident that occurred 11/09/2023. Documentation was reviewed. The preponderance of the evidence gathered during the investigation support non-compliance with standards.

Please sign and complete the ?plan of correction? and ?date to be corrected? for the violation cited on the violation notice and return it to me within 5 business days.

Based on review of documentation provided by the center and interview with center director, it was determined that staff roughly handled a child.
Evidence:
On November 9, 2023, S1 roughly handled a child by ?speaking in a negative tone to the child, and was standing on the child?s hair, restricting their movement.?
