Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 08/27/2024 and completed on 08/28/2024. The on-site inspection began at 9:30am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 77 children present and 19 staff. The inspector reviewed 10 children?s records and 10 staff records electronically on 08/28/2024. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/12/2024. A POC submitted after this date will not appear on the public website.

Based on a review of children?s records, the center did not ensure that each child?s record contained documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. Child #2, 2 years of age, had immunization records dated 09/19/2022 and 10/27/2023. Child?s first day of attendance was 09/19/2022. An updated immunization was required in March of 2023.
2. Child #9, 16 months of age, had one immunization record dated 08/08/2023 and 01/08/2024. Child?s first day of attendance was 08/21/2023. An updated immunization was required in July of 2024.
Plan of Correction: Child #2, updated records were on-site and not located in child's file. Updated records attached that show date of submission.

Child #9, parents were contacted and electronically submitted required updated immunizations.
