During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on October 27, 2025 and completed on October 27, 2025. The inspection started at 10:40am and concluded at 11:30am. There were no children present and 1 caregiver; the point total was zero. The inspector reviewed 1 child?s records and 3 caregiver/household member(s) records on-site and electronically on October 27, 2025.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date).
Violations
5Children's records shall be maintained with all required information.
Child 1's record did not contain the child's date of enrollment, address, the listed parent's work address, or documentation about any allergies the child has.
Caregivers shall undergo TB screenings at
least every two years from the date of the initial
screening, or more frequently if recommended
by a physician.
Caregiver 1's TB screening has been expired
for over one year.
Caregiver 1 has completed ten training hours.
There was no documentation of shelter-in-place procedures being practiced.
There was no documentation that lockdown procedures have been practiced.
