The program is required to request out-of-state background checks prior to employment for staff who have lived outside of Virginia within the past five years.

The record for Staff #2, who has been employed for more than 16 months, did not include documentation to show that an out-of-state sex offender check was requested from a state the employee resided in within the past five years.

The program is required to obtain documentation of a physical examination by or under the direction of a physician for each child before attendance or within 30 days after the first day of attendance.

The record for Child C, who has been enrolled for more than five months, did not contain documentation of a physical.

The program is required to ensure that each staff member submits documentation of a negative tuberculosis (TB) screening at the time of employment and the documentation must be dated within the last 30 calendar dates of the date of employment.

The TB screening on record for Staff #4 was dated more than four months prior to employment.

A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

The record for Child B contained documentation of food allergies in addition to emergency medication being observed on-site for the child?s allergies. There was no allergy care plan available on-site for Child B.

The program did not obtain all of the required on-site documentation for each staff member.

The record for Staff #2 did not include documentation of the name, address, and telephone number of a person to be notified in an emergency.

All staff who work directly with children are required to have current certification in cardiopulmonary resuscitation (CPR) within 90 days of employment. The training is required to include an in-person competency demonstration.

Staff #1, who has been employed for more than four months, did not have current CPR certification that included an in-person competency demonstration.

(Repeat Violation) Staff who work directly with children are required to annually attend at least 16 hours of training to include the department?s health and safety update course.

The record for Staff #2 did not contain documentation of completing the health and safety update training. The training was past due by more than four months.
