Inspector Notes
An unannounced monitoring inspection was conducted 9/13/2023 from 3:45pm through 4:45pm. There were 16 children directly supervised by 2 staff and the group was within ratio. The physical plant, programming, menu, 2 staff records, 6 children?s records, 1 medications, emergency drills, emergency supplies, and policies were inspected. Children were observed participating in quiet table top activities and eating snack which consisted of orange slices and a cheese stick. One staff on site has CPR/First Aid certification but no staff were present with Daily Health Observation training or Medication Administration Training. Areas of non-compliance are identified in the Violation Notice.

Violations
4Evidence: Neither of the two staff members present at the time of inspection had current MAT certification and there was an emergency medication on site.
Evidence: Neither of the two staff present at the time of inspection have DHO training.
Evidence: There was only one staff, Staff #2, who had current CPR and first aid certification at the time of inspection.
Evidence: Child #6 has a diagnosed food allergy but did not have a written allergy care plan on file.
