Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A renewal inspection was initiated on 01/20/2022 and concluded on 02/02/2022. The director was contacted by telephone to initiate the inspection. There were 37 children present and 13 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-770-60-C-2
Based on a record review the center did not ensure all staff had the required out-of-state background checks.
1. Staff #1 (hired 1/27/21) indicated living outside the state of Virginia within the past 5 years and did not have documentation of a background check, central registry check or sex offender check from that state.
2. Staff #3 (hired 10/06/20) indicated living in 2 states outside of Virginia within the past 5 years and did not have documentation of a background check, central registry check or sex offender check from that state from one and the other did not have a criminal record check or sex offender check.
Plan of Correction: The following corrective action was provided by the director during the exit interview:
All out of state background checks will be obtained and filed in each staff record.
Standard 8VAC20-780-270-A
Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
There was peeling and paint and rust in one of the bathroom stalls. A reddish brown substance rubbed off and onto the director's finger when touched.
Plan of Correction: The following plan of correction was provided by the director during the exit interview:
The rusted area and peeling paint will removed.
