Inspector Notes

An unannounced monitoring inspection was completed 02/10/2025. The on-site inspection began at 1:15pm and ended at 3:30pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 68 children present and 9 staff. The inspector reviewed 5 children?s records and 4 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
Centers may not employ staff that do not have the required background checks and/or repeat checks every 5 years.

Staff B's record did not have documentation of a repeat fingerprint since the last one that was date over 5 years ago on 08/19/2019.
Plan of Correction: This was an oversight. Staff B will be sent this week to obtain updated fingerprints.
Standard 22.1-289.035-B-1
Staff are required to provide a sworn statement or affirmation disclosing whether he has ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.

Staff A and Staff C did not have the most current sworn statement of affirmation that includes disclosing having lived outside the state of Virginia within the last 5 years.
Plan of Correction: An old form was used for some of the staff. New sworn statements will be printed for all staff to ensure everyone has the most recent form.
Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

The center's last fire inspection was dated 10/10/2023.
Plan of Correction: The fire Marshall will be contacted to schedule an inspection as soon as possible. the phone call will be documented in the record.
Standard 8VAC20-780-340-D
In each grouping of children at least one staff member who meets the
qualifications of a program leader or program director shall be regularly present.

Staff C who was assigned to supervise the school-age classroom did not have documentation of having met the qualifications of a program leader.
Plan of Correction: Administration will work on Staff C obtaining the required training and experience that will qualify her as a program leader. Her qualifications will be documented in her record.
