Each building that was built before 2015 and that houses a child day program or serves preschool age children shall be equipped with at least one carbon monoxide detector. The center did not have a carbon monoxide detector.
Plan of Correction: The center will obtain a carbon monoxide detector for the building.
Standard 8VAC20-780-160-C
Staff are required to obtain a tuberculosis screening or test at least every two years from the date of the first initial screening or testing. Staff #3 did not have a current tuberculosis screening at least 2 years since the last one.
Plan of Correction: The director had already provided staff #3 with paperwork to obtain her repeat TB test. She will follow-up to ensure it is done by the end of next week.

The center is required to provide documentation of annual approval from the health department. The director did not have documentation of having a health inspection and stated she was in the process of scheduling an inspection.
Plan of Correction: The director has been working on scheduling the health department to come out for an inspection. She will contact them again today.
