Inspector Notes
An unannounced in person complaint inspection was conducted on 7/22/2024 from 10:30 am until 11:00 am. in response to a complaint received by the Fairfax Licensing office in regards to supervision. Interviews were conducted, 6 children's records, 2 caregiver records, and attendance records were reviewed. The information gathered today did not support the claims made in the complaint and the complaint is deemed "not valid".

The provider is required to maintain children's records containing required information. Child 6's record did not contain the name and contact information for two emergency contacts, the place of employment and contact numbers for each listed parent's place of employment, proof of the child's age and identity, or a written authorization for emergency medical care.
Plan of Correction: Before a child begins attending Day care I will ensure that the parents give me the forms with all the information required to maintain the current registration. In this case I proceeded to request it from the father and he gave it to me, so in the future Silvia will is the person responsible for this function and will proceed to follow the established rules, with more care and attention.

The provider is required to inform parents, in writing, whether the family day home carries liability insurance. Child 6's record did not contain documentation that the information was provided.
Plan of Correction: The father had in advance the information for the liability insurance that I provided, proceeded to sign the form required.
