Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/15/2024 and completed on 10/15/2024. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 12. The inspector reviewed 4 children?s records and 3 caregiver/household members records [on-site].
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/22/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Caregiver 1's subsequent Tuberculosis screening was due on 3/25/2022.
Plan of Correction: I proceeded to make an appointment for my Tuberculosis test update.
Standard 8VAC20-800-50-C
Information contained in a child's record shall be privileged and confidential. The provider shall not distribute or release information in a child's record to any unauthorized person without the written consent of the child's parent.

A confidential page from a previous licensing inspection was posted. The page contains identifying information about children in care.
Plan of Correction: The confidential page from a previous licensing inspection was removed immediately, and in the future I will make sure not to post it.
Standard 8VAC20-800-730-D
Medication, shall be kept in a locked place using a safe locking method that prevents access by children.

A tube of prescription topical ointment was observed to be unlocked in the child care area.

Standard 8VAC20-800-780-B
Annually, the provider shall review the emergency contact information with the parent and obtain the parent's signed acknowledgement of the review.

Child 1 and Child 3's record were due to be updated in September 2024.
Plan of Correction: The parents of children 1 &3 at the time of pick up them from the daycar proceeded I sign the review the emergency contact info [sic].
Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations for a child every six months until the child is two years old.

Child 4's (aged 3.5 years) immunization record was last updated when the child was 11 months old.
Plan of Correction: I proceeded to request the updated vaccination certification from the father and he gave it to me when he came to pick up the child.

When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated.

On the date of inspection, the posted menu was dated for 109/7/24-10/11/24.
Plan of Correction: As soon as the supervision ended I proceeded to rectify the correct dates of the daily meal menus.
Standard 8VAC20-800-960-E
Bottles shall be refrigerated and labeled with the child's full name and the date, if more than one infant is in care.

One bottle was observed without a name or date labeled. One bottle was observed with only the child's name labeled.
Plan of Correction: I immediately put the name and date on the bottles as required by the regulations.
