Inspector Notes

An unannounced renewal inspection was conducted on 1/5/2024 from 10:25 am to 1:05 pm. There were 25 children in care, ages seven months to five years, in four groups under the care of five staff. The Licensing Inspector reviewed four children?s records, three staff records, and other program records. The children were observed in free play indoors and snack.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the facility was not maintained in a clean, safe, and operable condition.

EVIDENCE:
The floor in the bathroom next to the kitchen is damaged and is a trip hazard. In two areas inside the bathroom near the doors, the flooring is peeling up and missing pieces.
Plan of Correction: The provider will keep asking the landlord to fix this problem.
