Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/26/2024 and completed on 8/26/2024, as a part of the licensure period. The on-site inspection began at 10:20AM and ended at 11:49AM. The inspector reviewed compliance in the areas listed above. There were 18 children present and 5 staff. The inspector reviewed 4 children?s records and 4 staff records on-site. This inspection included ? document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, and observations (this would include observations of photos, recordings, etc.)

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Licensing inspector observed a hole in the wall approximately 1 inch in diameter in the preschool class and a toilet tank lid that was cracked in half in the toddler class bathroom during the inspection on 8/26/2024. Children were not observed in the areas but they were accessible to children.
Plan of Correction: Provider will make repairs to facility.
