Inspector Notes

An virtual renewal inspection was conducted today with the provider, who was present with 5 children, age ranging from 5 months to 3 years for a total of 16 points. One child had emergency medication onsite. The provider's Medication Administration Training for emergency medication was obtained on 3/15/21. The provider and the assistant's First Aid/CPR certification is through ASHI and is due to expire on 1/9/23. Three children's files were reviewed.
The violations issued are outlined on the violation notice.

Standard 22VAC40-665-410-A-2
Based on interview, it was determined that the provider has not practiced shelter in place drills twice a year as required. There was no documentation of shelter in place drills practiced for the 2021 year.
Plan of Correction: I will do shelter in place drills twice a year and keep documentation.
Standard 8VAC20-770-40-D-5-a
Based on interview, it was determined that a 16 year old household member (A) did not have a central registry results on file. This background check is required for household members when they turn 14 years of age.
Plan of Correction: I will get this background checks and send you the results.
Standard 8VAC20-800-180-A
Based on interview and a review of the tuberculosis test/screenings, it was determined that two household member's tuberculosis tests/x-ray were more than 2 years old.
Evidence:
Person B's tuberculosis test is dated 11/13/19 and Person C's tuberculosis x-ray is dated 12/19/19.
Plan of Correction: We will obtain new tuberculosis test/x-rays.
