Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 9/11/2024, as a part of the provisional licensure period. The on-site inspection began at 9:48am and ended at 10:25am. The inspector reviewed compliance in the areas listed above. There were (7) children present and (2) caregivers; the point total was (18). The inspector reviewed (7) children?s records and (2) caregivers records on-site. This inspection included document review, tour of the facility and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-90-C
Based on review of seven children's records, the provider did not obtain documentation of updated immunizations once every six months for children under the age of two years.
Evidence:
1. Child #3 (under 2 years old) had an immunization record dated 12/01/2023 and needed an updated immunization record on file.
Plan of Correction: I will get the immunization from the parent after their appointment on Friday.
