Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on o4/19/2021 and concluded on 04/23/2021. A self-reported incident was received by the department regarding allegations in the areas of Background checks. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued.

Based on a staff interview and record review, the center did not ensure documentation of staff fingerprint results were completed prior to employment.
Evidence:
Staff #1 hire date 3/22/2021 did not have documentation of the fingerprint results prior to the first date of employment.
Staff #2 confirmed staff #1 worked from 3/22/2021 to 4/12/2021 at the center.
