Inspector Notes

An unannounced monitoring was conducted on October 28, 2022 from approximately 12:00pm- 2:06pm. There were 19 children present upon arrival with 6 staff members. Additional administrative staff were present. Children were observed napping. Three staff records and three children's records reviewes . Licensing Inspector reviewed indoor areas, supervision, activities, equipment, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the licensee did not ensure staff had tuberculosis screening test within 30days prior to employment.

Evidence:
Staff #2 (employment date 09/14/2022) did not have tuberculosis screen 30days prior to employment. Tuberculosis test for staff #2 was completed 09/20/2022.

Based on record review and staf finterview, the licensee did not ensure a health inspection was conducted annually.

Evidence:
The most recent Health inspection was 06/24/2021. Staff #1 did state and provided document the center contacted Health department for inspection in June. Center owner was able to schedule inspection while inspector was present.

Based on record review and staff interview, the licensee did not ensure the allergy list was dated.
