Inspector Notes

An unannounced monitoring inspection was conducted from 12:00pm- 1:!2pm. At the time of entrance, 33 children were in care with 8 staff members present. Additional staff were available to assist. The sample size consisted of 3 children?s records and 3 staff records. Children were
observed during naptime.

Information gathered during the inspection determined noncompliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff interview and record review, the licensee did not ensure obtain a copy of the results of a a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence: Staff #3 hire date 11/30/2022 lived in Florida in the past five years the center was unable to provide documentation that the out of State Child Abuse and Neglect Search Request results.

Based on staff interview and record review, the licensee did not ensure a negative tuberculosis screening results were submitted at the time of employment and prior to coming into contact with children.
