An unannounced complaint investigation was conducted on May 22, 2024 7:40 am-9:04 am . The inspection concluded on May 22, 2024.The complaint was in regards to staffing and supervision, special care provision and emergencies, programs, physical plant and administration. Staff were
interviewed, observations were conducted and documents were reviewed.

There was not a preponderance of evidence in regards staffing and supervision, administration, programs and special care provision and emergency gathered during investigation to determine if the center was not in compliance.

There was a preponderance of evidence gathered during the investigation to determine in regards to physical plant the center was not in compliance. The complaint for physical plant is deemed VALID.

Based on observation, the licensee did not ensure areas and equipment of the center inside was maintained in a clean, safe and operable condition.

Evidence:
The walls in the infant room, had multiple areas on the wall and door with orange , tan and brown residue. When wiped with a paper towel brown residue would come off on the paper towel.

Center staff confirmed during tour of infant room.
