Inspector Notes
A renewal inspection was initiated and concluded on 02/01/2024. The inspector arrived at 12:15 PM and departed at 1:45 PM. There were 14 children present, ranging in ages from 2 yrs to 5 yrs, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.
Violations
3Evidence:
1. The sworn statement or affirmation form for staff 4 indicated that staff 4 has lived in another state within the past five years but did not indicate the state in which the staff person lived.
2. Staff 1 confirmed that the sworn statement was not complete/correct for staff 4.
Staff 4 has not lived outside of Virginia within the previous five years. Staff 4 checked the incorrect box on the sworn statement. This has been corrected.
Evidence:
The record provided for child 2 indicated that child 2 began attending on 6/23/2022. The record did not include documentation of immunizations.
The parent was immediately notified that documentation of immunizations is required. The child did have the required physical examination.
All children's records will be re-checked to ensure that they are complete with documentation of immunizations.
Evidence:
Staff records included documentation of CPR certification but did not include first aid certification.
All staff have first aid certification. They were issued the incorrect certificates following the combined first aid/CPR training.
