Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.
An unannounced complaint inspection was initiated on 12/10/2021 and concluded on 12/13/2021 regarding allegations of non-compliance with ratio. The inspector was on site on 12/10/2021 from 12:00-12:45 pm. There were 38 children present, ranging in ages from 4 months to 9 years, with 5 staff supervising. The inspector conducted observations and interviews on site. Additional documentation was requested and reviewed remotely.
The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. 2Evidence:
1. On 12/10/2021, the infant room had 5 children present with 1 staff. The children ranged in age from 4 months to 16 months.
2. Administration acknowledged that the classroom was not following the required staff to child ratio.

Evidence:
1. At 12:00 on 12/10/2021 there were 15 children ranging in age from 4 to 9 years with 1 staff. One additional staff was needed to meet the required ratio of one staff to ten children.
2.The staff stated that the school aged children had early release and had joined the preschool class until the director returned. The director returned around 12:30.
3. Administration acknowledged that the classroom was out of compliance with ratio requirements.
