Inspector Notes
This inspection was conducted by licensing staff partially using an alternate remote protocol, including telephone contacts, documents review, and interviews. The inspector was on site on 3/15/2022 from 9:12-9:56 am.
An unannounced inspection was initiated on 3/15/2022 and concluded on 3/21/2022 in response to a complaint that was received by the department on 03/10/2022 regarding allegations in the area of staff to child ratio requirements, procedures for emergencies, termination policies, and behavioral guidance.
The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. There was also a violation found during the inspection regarding over the counter skin products. See the violation notice for more details. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
3Evidence:
1. The Pre kindergarten classroom (4's) was observed to not be within the sight of a staff member for 1.5 minutes when the staff stepped outside of the classroom to speak to an individual at the door.
2. Administration acknowledged that they stepped outside of the classroom to talk to a parent.
Evidence:
1. On 3/15/2022, 13 children ranging in age from 2-3 years were observed with 1 staff from 9:15 am-9:48am.
2. Administration acknowledged that the classroom was out of compliance with ratio.
Evidence:
1. Two diaper ointments were observed in an unlocked cabinet in a children's bathroom. During the inspection, children were observed utilizing the area where the unlocked diaper creams were stored.
2. Administration acknowledged that the child proof latch was not properly secured.
