Inspector Notes
An unannounced monitoring inspection was initiated on 5/18/2022 in response to a self-reported incident that the Department received on 5/16/2022 regarding forbidden action. The inspector was on site at the center on 5/18/2022 from 10:30 am-11:30 am. An interview with staff was completed, observations were made in the center, and documentation was reviewed and requested. The inspector completed additional interviews remotely on 6/2/2022 and 6/6/2022
The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the report is determined to be not valid. Though not a part of the original report, there were violations found during the inspection regarding immunizations and physical examinations for children. See the violation notice for more details.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
3obtain documentation that 1 child had received the immunizations required by the State Board of Health before the first date of attendance as required.
Evidence:
1. The record of child #2 (DOE: 6/28/2021) did not contain documentation of an immunization record.
2. Administration acknowledged that the immunization record was missing.
Evidence:
1. The record of child #1 (DOE: 6/28/2021) contained an immunization record dated 6/11/2021. The child (age 1) was due for an update in December. The record did not contain the update.
2. Administration acknowledged that the immunization update was missing.
Evidence:
1. The record of child #2 (DOE: 6/28/2021) did not contain a physical record.
2. Administration acknowledged that the physical was missing.
