Inspector Notes
A Subsidy Health and Safety inspection was conducted on 1/9/2023 to review supplemental health and safety requirements. There were 31 children present with 7 staff supervising.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on a review of five (5) staff records and interview, the vendor did not ensure that one (1) staff who worked directly with children annually completed the department's health and safety update course as required.
Evidence:
1. The record of staff #2 (date of employment: 9/23/2019) did not contain a 2022 annual health and safety training certificate.
2. The vendor acknowledged that it had not been completed by staff #2.

Plan of Correction: Staff #2 will complete the annual health and safety training. Administration will check for annual training every three months to make sure the staff is in compliance with license standards.
