During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 7/28/2025. The on-site inspection began at 9:29 am and ended at 12:08 pm. The inspector reviewed compliance in the areas listed above. There were 59 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/04/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Staff #3's updated screening was due over 2 months ago. The updated screening had not yet been completed.
Plan of Correction: Administration will posted all staff TB screening in an easy access area so staff knows when its time to update TB screening.
Standard 8VAC20-780-350-B-5
The required ratio for children school age eligible up to nine years is one staff per 18 children.

Staff #6 was observed supervising a group of 22 school aged children for approximately 20 minutes before an additional staff member arrived.
Plan of Correction: Staff will track attendance more accurately throughout the morning drop off time to determining if ratios are met according to license standards.

Children who are not toilet trained may not use portable wading pools.

On 7/28/25, non-toilet trained children were observed playing in portable wading pools.
Plan of Correction: Administration will find alternative activities for children who is not toilet trained so they can participate in water activities.

At least one staff in each classroom or area where children are present shall have current certification in CPR.

Staff #3 was alone with a group of 10 preschool aged children and did not have a current CPR certification. Staff #4 was alone with a group of four infants and did not have a current CPR certification.
Plan of Correction: Administration will have staff complete a CPR training within 15 business day. Administration will posted all certification in classroom to remind staff when to renew their certification.

At least one staff in each classroom or area where children are present shall have current certification in first aid.

Staff #3 was alone with a group of 10 preschool aged children and did not have a current first aid certification. Staff #4 was alone with a group of four infants and did not have a current first aid certification.
Plan of Correction: Administration will posted in each class the staff certifications so the staff can be responsible for updated all certification before they expired.
Staff will complete a first aid training in 15 business days.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the required information.

Child #6 had an injury report that was missing the time of parent notification and any future action to prevent recurrence of the injury. Child #7 had an injury report that was missing the time of parent notification and any future action to prevent recurrence of the injury. Child #8 had an injury report that was missing the time of parent notification. Child # 9 had an injury report that was missing the time of parent notification and any future action to prevent recurrence of the injury.
Plan of Correction: Administration will appoint one staff to review all injury report to make sure that all documentation is filled out completely before putting the reports in the injury file to be stored.

Bottles shall not be propped or used while the child is in their designated sleeping location.

On 7/28/25, staff #4 fed an infant a bottle in their crib.
Plan of Correction: Administration will review license standards on infant care with infant teachers. Staff will do a refresh training on infant care.
