During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 3/19/2026 and completed on 3/25/2026 in response to a self-report received by the licensing office on 3/12/2026 relating to forbidden action and behavioral guidance. The inspector reviewed 1 child record and 1 staff record on-site. This inspection included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/01/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-E
The licensee shall include ensuring that the center's activities and services are maintained in compliance with the standards and the center's own policies and procedures that are required by these standards.

The center's discipline policy to include acceptable and unacceptable discipline states that staff will model appropriate behavior and redirect children and restricting movement through binding or typing is forbidden. Staff #1 stated that on 3/09/2026, Child #1 repetitively placed their hands in their mouth which resulted in the child's saliva coming into contact with undesired surfaces and other children. To prevent the behavior, Staff #1 placed tape around the child's hands for approximately 6 minutes before it was removed. Administration acknowledged that the actions of Staff #1 were a violation of the center's policies.
Plan of Correction: The staff member involved was terminate after the investigation. In addition, all staff members will participate in a mandatory review training on the center discipline policy, licensing regulations, and appropriate methods for addressing challenging behaviors.

Moving forward, administration will be notified by staff immediately of any challenging behavior concerns so that appropriate behavior plans can be developed without the use of prohibited practices.

Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On 3/9/2026, Staff #1 wrapped tape around the hands of Child #1, age three, for approximately 6 minutes. In Interview, Staff #1 stated that they placed tape around each of child's hands to prevent the child from sticking their hands in their mouth.
Plan of Correction: All staff will be immediate retraining on appropriate behavior guidance techniques, including redirection, positive reinforcement, close supervision, and providing alternative sensory or comfort items when children engage in behaviors such as hand-to-mouth actions.
Administration will increase classroom monitoring. Staff #1 was dismiss from the position.

Standard 8VAC20-780-410-1
The following acts are forbidden: physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment.

On 3/09/2026, Staff #1 restricted the movement of Child #1 when they wrapped tape around each of child's hands for approximately 6 minutes. The tape was placed from the base of the fingers up to the child's wrist.
Plan of Correction: To ensure immediate compliance, the staff member involved has been removed. Administration will give all staff clear identification of forbidden actions, with an emphasis on maintaining full compliance at all times. Documentation of this training will be maintained and reviewed regularly.
