An unannounced complaint investigation was conducted on 3/1/22 in response to a complaint received in the licensing office regarding allegations of accident/incident reports not being maintained and unsafe conditions on the playground. Based on observation, the allegation regarding the playground was determined to be valid and violations were cited. There was insufficient corroborating evidence to support the allegation regarding accident/incident reports not being maintained, therefore that allegation was determined to be not valid. Based on observation, the center did not ensure that equipment, inside and outside, was maintained in a safe and operable condition. Evidence: On the playground equipment there were several pieces of wood that were splintering, cracking, or otherwise deteriorating that posed a splinter or scratching hazard.
Plan of Correction: The wood repairs were made to the playground on 3/4/22. The company will return to stain the wood on 3/6/22.
Standard 8VAC20-780-330-B
Based on observation, the center did not ensure that the required amount of resilient surfacing was under playground equipment with moving parts or climbing apparatus to create a sufficient fall zone. Evidence: The pea gravel under the swings measured approximately four inches deep. Each swing had one rubber mat (approximately 5 feet long) underneath that was placed on top of the pea gravel. The requirement for swings is for six inches of resilient surfacing that is twice the height of the swing beam in the front and back of the swing.
Plan of Correction: We had a previously scheduled delivery for more pea gravel on 3/4/22. We will be sure to rake it to the proper measurements in all the fall zones.
