Inspector Notes

An unannounced monitoring inspection was conducted on 7/11/22 from 11:20am to 2:15pm with the General Manager. There were 32 children in care, ranging in age from two-years to five-years-old, supervised by six staff. The children were observed finishing lunch, washing hands, getting ready for nap, and napping. Five child records and four staff records were reviewed. The Center has seven staff with current certification in CPR and First Aid, and all staff trained in Daily Health Observation Training. One medication and authorization form were reviewed and the center has four staff certified in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 5/26/22 and the most recent Health Inspection on file was dated 10/28/21. Lunch served was chicken and rice with peas, pears, and milk. Based on observation, the center did not ensure that areas of the center were maintained in a safe condition. Evidence: The wall behind the cots in the Preschool classroom contained multiple areas where the wall material/paneling was crumbling.
Plan of Correction: Wall was repaired on 7/12.
Standard 8VAC20-780-70
Based on review of five staff records, the center did not obtain all of the required documents for staff records. Evidence: The records for Staff #1 and Staff #2 did not contain documentation to demonstrate that the individual possesses the education or certification required by their job position.
Plan of Correction: Staff files had certificates separated and labeled clearly from annual training certificates. Staff #1 had certificates, but not clearly separate, Staff #2 had only been employed for 3 weeks and was finishing her 24 hours of training. Transcripts have been received and files have been updated.
